When I built my adapter I had similar problems. I had built the board using rework wire since I didn't have any board etching equipment. It turned out that I had a few intermittent solder joints, and when I carefully checked out each connection and resoldered them it became very reliable. I am also using VAG-COM on a 96 Audi A4, which only uses 3 wires (No-L) I am using a desktop PC.
I don't think the choice of Optos is very critical. When I couldn't find the ones that were originally listed, I crossed it to a generic unit, (NTE-3098).
I am using about 50 feet of cable between the adapter and the car, so cable length doesn't seem to be critical.
